Аннотация: В настоящее время лоскуты передней брюшной стенки (ПБС) – метод выбора в реконструкции молочной железы. Классическому TRAM лоскуту приходят на смену сберегающие мышцу аналоги. Для снижения риска ослабления ПБС были разработаны аутотрансплантаты, в которые входили только кожа, подкожная клетчатка и сосуды. Эти лоскуты оптимальны для реконструкции молочных желез, но, к сожалению, их практическое использование затруднено вследствие значительных технических сложностей, связанных с необходимостью наложения сосудистого анастомоза, что требует владения микрохирургической техникой. Анатомическая вариабельность сосудистого русла также ограничивает возможность их применения. Компьютерно-томографическая ангиография (КТА) ПБС – метод, который с недавнего времени используют для обследования больных, готовящихся к операции восстановления молочных желез после мастэктомии лоскутом ПБС на микрососудистых анастомозах, для определения топографических особенностей нижней эпигастральной артерии (НЭА). В статье представлен сравнительный анализ работ, посвященных предоперационной оценке особенностей строения сосудистого русла ПБС. Сейчас разработаны режимы КТА, которые дают возможность получить хорошую визуализацию НЭА и ее ветвей практически в 100% исследований и снизить лучевую нагрузку на пациента.
