Аннотация: Цель. Изучение отдаленных клинических результатов после стентирования эндопротезов с лекарственным покрытием - пролонгированных стенозов, частоты развития рестеноза (РС) по данным ангиографии, динамики эндотелизации и других морфологических показателей по данным внутрисосудистого ультразвукового исследования (ВСУЗИ). Материалы и методы. В исследовании участвовали 220 пациентов с клиникой стенокардии и/или признаками ишемии миокарда, которым были имплантированы стенты с рапамициновым покрытием. 174 больным в 1-й год и 82 пациентам в течение 2-го года после имплантации эндопротезов была выполнена контрастная коронаровентрикулография. В течение 1-го года двойную антиагрегантную терапию получали 198 (90%) пациентов, в течение 2 лет - 21 (9,5%) больной. Результаты. Общий ангиографический успех составил 89,5%. У 44% пациентов эндоваскулярные вмешательства проводили с защитой боковых ветвей. Безуспешность операции в большинстве случаев была обусловлена технической невозможностью провести стент через измененный сегмент коронарной артерии - 5%, в 1,8% наблюдений выявлено неполное расправление эндопротеза, в 2,7% - окклюзия боковой ветви. Выводы. По данным ВСУЗИ к концу 1-го года наблюдения 40% стентов с лекарственным покрытием имели полную эндотелизацию, к концу 2-го - уже 91%. Двойную антиагрегантную терапию в течение 1-го года получали 99,1% больных. Суммарное количество коронарных событий (летальность + инфаркт миокарда + рецидив стенокардии либо повторная реваскуляризация) в этот период было достоверно ниже по сравнению со 2-м годом наблюдения, в течение которого такое лечение назначалось только 9,6% пациентов.
