Аннотация:

Введение: обзор посвящен клиническим результатам применения при внутрипеченочной холангиокарциноме рентгеноэндоваскулярных вмешательств: химиоинфузии, химио- и радиоэмболизации печеночной артерии, предоперационной эмболизации правой ветви воротной вены.

Цель: оценить и сравнить эффективность методов внутрисосудистой терапии внутрипеченочной холангиокарциномы.

Материалы и методы: в статье представлен анализ 50 литературных источников в ведущих отечественных и зарубежных научных журналах.

Результаты: установлено, что внутриартериальные методы лечения обладают примерно одинаковой клинической эффективностью. Химиоинфузия - технически несложный и действенный метод лечения, перспективы которого связывают с созданием новых химиопрепаратов и терапевтических схем. Химиоэмболизация наиболее эффективна при гиперваскулярной форме холангиокарциномы. Требует изучения вопрос об ее использовании в неоадьювантном режиме, даже в резектабельных случаях она способствует снижению биологической активности опухоли. Радиоэмболизация (РЭ) эффективно замедляет рост холангиокарциномы и хорошо переносится больными, однако отдаленные результаты несколько уступают таковым при инфузии и эмболизации. Процедура представляется технически сложной и требующей дорогостоящей логистики. При решении этих проблем РЭ может стать одним из значимых методов лечения холангиокарциномы, особенно при резистентности опухоли к другим методам терапии.

Предоперационная эмболизация воротной вены рутинно используется в клинической практике. Однако операции, выполненные после этой процедуры, составляют лишь 3-6% от всех резекций печени. Логичным выглядит более широкое применение этой технически простой и безопасной методики.

Выводы: в лечении холангиокарциномы следует использовать комбинированный подход с применением хирургических, рентгеноэндоваскулярных и других методов противоопухолевой терапии: это позволяет расширить возможности лечения больных и добиться улучшения отдаленных результатов.
