Аннотация

Актуальность темы: гилюсная холангиокарцинома (опухоль Клацкина) - редкое и тяжело протекающее злокачественное новообразование проксимальных внепеченочных желчных протоков, с неблагоприятным прогнозом, отличающееся медленным перидуктальным инфильтративным ростом и поздним метастазированием, приводящее к смерти больного чаще всего из-за локальных осложнений. Основным методом лечения этого заболевания считается хирургический, однако большинство (до 70-80%) пациентов к моменту установления диагноза являются неоперабельными в силу местной распространенности заболевания. Кроме того, частота локальных рецидивов даже после радикальных операций достигает 80% в течение 7 лет В этой связи для воздействия на эту опухоль был предложен ряд локорегионарных технологий, одной из которых является эндобилиарная фотодинамическая терапия (ФДТ).

Цель исследования: привести преклиническое обоснование целесообразности применения ФДТ у больных опухолью Клацкина: описать физико-химические и биологические механизмы действия метода и обобщить данные экспериментальных исследований, что подготовит читателя к восприятию второй части обзора, основанной на анализе результатов клинических работ и имеющей более практическую направленность.

Материал и методы: проанализировано 63 отечественных и зарубежных литературных источника.

Заключение: внутрипротоковая фотодинамическая терапия продемонстрировала свою безопасность и эффективность во многих экспериментальных исследованиях и может с успехом применяться в клинической практике.
